Never give Antabuse (disulfiram) to a patient in a state of alcohol intoxication or without the patient's full knowledge.  The patient should not take disulfiram for at least 12 hours after drinking.  A reaction may occur for up to 2 weeks after disulfiram has been stopped.

Alcohol abuse and dependence are associated with 100,000 deaths annually in the United States (US), cause 30 percent of all traffic fatalities, and affect approximately 10 percent of Americans at some point in their lives.  The prevalence of unhealthy alcohol use in the US in estimated to be between 7 and 20 percent for outpatients.  A 2002 representative survey of 43,000 US adults found the lifetime prevalence of alcohol dependence to be 12. 5 percent; only 24 percent of those reporting alcohol dependence were ever treated.  Primary care physicians play many roles in the management of patients with unhealthy alcohol use.  Structured physician advice can substantially decrease the amount of alcohol consumed and decrease the number of episodes of binge drinking.  The primary care physician also needs to identify the problem, present the diagnosis, initiate treatment, and participate in the long-term continuum of care for the alcohol abusing or dependent patient. .
